A Success in Pregnancy after Bilateral Total Adrenalectomy

Abstract

Nowadays pregnancy in a patient with adrenocortical insufficiency is no longer regarded as a serious medical problem partly because the hormone replacement therapy and other measures in the treatment of adrenocortical insufficiency have achieved marked improvement and partly because the delivery can usually be per vias naturales and need not be induced before term as the fetus is not abnormally large and does not show an increased intrauterine mortality towards the end of pregnancy. In this case, however, a Caesarean section was performed in a patient given bilateral total adrenalectomy for Cushing syndrome in view of the fact that she was a primara with breech presentation. The result was satisfactory with delivery of a healthy female infant of 3.1kg of weight with no congenital anomaly and with no postpartum complication in the mother. It seems that this case is the first successful pregnancy and delivery in a patient with adrenocortical insufficiency in Korea. We could not find any other case reported in the Korean literature.
